Definition 
 
Anaphylactic is a collection of symptoms that result from an acute reaction to a foreign substance to a person who previously had the sensitization (immediate / hypersensitivity reactions indirect immunity). 
 
Etiology 
<ul>
<li>Because drugs indirect histamine reaction that usually follows the injection of the drug weight, serum, the x-ray contrast media.</li>
<li>Certain foods, insect bites.</li>
<li>The reaction can sometimes idiopathic / immunologic abnormalities manifestations.</li>
</ul>
Symptoms 
<ul>
<li>Cardiovascular: tachycardia, hypotension, shock, arrhythmia, palpitations.</li>
<li>Respiratory tract: rhinitis, sneezing, itching of the nose, bronchospasm, hoarseness, shortness, apnea.</li>
<li>Gastrointestinal: nausea, vomiting, abdominal pain.</li>
<li>Skin: pruritus, urticaria, angioedema, skin pale and cold.</li>

Nursing Diagnosis for Anaphylactic Shock 
 
1. Impaired gas exchange related to ventilation perfusion imbalance. 
characterized by: shortness of breath, tachycardia, flushing, hypotension, shock, and bronchospasm. 
 
2. Altered tissue perfusion related to decreased blood flow secondary to vascular disorders due to anaphylactic reactions. 
characterized by: palpitations, skin pale, cold acral, hypotension, angioedema, arrhythmias, ECG features horizontal and inverted T waves. 
 
3. Ineffective breathing pattern related to the swelling of the nasal mucosa wall 
characterized by: shortness of breath, breath with the lips, there rhinitis. 
 
4. Acute pain related to gastric irritation 
characterized by: abdominal pain, looked grimacing while holding stomach. 
 
5. Impaired skin integrity related to changes in circulation 
characterized by: swelling and itching of the skin and the nose, there are hives, urticaria, and runny nose.

 Etiology: 
 
Acute tonsillitis is caused by germs Group A beta-hemolytic streptococcal, pneumococcal, viridans streptococci and Streptococcus pyogenes. Viruses sometimes also be the cause of this disease. Tonsillitis is often occur suddenly in children with increase in temperature 1-4 degrees Celsius. 
 
 
Pathophysiology: 
 
Transmission of the disease occurs through droplets. Germs infiltrating epithelial layer, then when germs eroded, then the lymphoid tissue superficial to react, damming occurs inflammation with infiltration of polymorphonuclear leukocytes.

Clinical Manifestations: 
 
Group A streptococcal tonsillitis should be distinguished from diphtheria, non-bacterial pharyngitis, other forms of bacterial pharyngitis and mononucleuosis infectious. Symptoms and signs are found in acute tonsillitis include body temperature rose to 40 ° C, sore throat and pain when swallowing, breathing the smell, the sound will become hoarse, high fever, a sense of lethargy, pain in the joints, no appetite and ear pain. On examination will also appear swollen tonsils, hyperemia and detritus are shaped follicles, lacuna will be covered by a pseudo membrane. Submandibular gland swelling and tenderness. 
 
 
Complications: 
 
Acute otitis media (kids), abscess parafaring, toxemia, septicemia, bronchitis, acute nephritis, myocarditis, and arthritis. 
 
 
Management : 
 
The treatment is performed in patients with tonsillitis usually with self-care and the use of antibiotics. Surgery is only performed if it reaches tonsillitis that can not be handled alone. 
 
1. Self care 
If tonsillitis patients attacked because the virus should let the virus goes away by itself. For one or two weeks the patient should rest, drink hot beverages also consume liquid cooling. 
 
2. Antibiotics 
If tonsillitis is caused by bacteria, antibiotics play a role in the healing process. Oral antibiotics should be eaten for at least 10 days. 
 
3. Surgery 
Tonsilektomy usually performed on children if the child has tonsillitis during seven times or more in a year, the child has tonsillitis five times or more in two years, the tonsils to swell and result in difficulty breathing, abscesses.

Bell's palsy is a form of temporary facial paralysis resulting from damage or trauma to the facial nerves. The facial nerve-also called the 7th cranial nerve-travels through a narrow, bony canal (called the Fallopian canal) in the skull, beneath the ear, to the muscles on each side of the face. For most of its journey, the nerve is encased in this bony shell. 
 
Symptoms of Bell's Palsy :

<li>Sudden weakness or paralysis on one side of the face that causes it to droop. This is the main symptom. It may make it hard for to close the eye on that side of the face.</li>
<li>Eye problems, such as excessive tearing or a dry eye. </li>
<li>Drooling.</li>
<li>Pain in or behind the ear.</li>
<li>Loss of ability to taste.</li>
<li>Numbness in the affected side of your face.</li>
<li>Increased sensitivity to sound.</li>

Causes of Bell's Palsy : 
 
Temporary weakness or paralysis of the muscles in one side of the face. It is the most common cause of facial paralysis. 
 
Other causes of facial paralysis include:

<li>Congenital facial palsy – children born with facial weakness.</li>
<li>Injury from surgery – which is most common during surgery of the parotid gland and neck.</li>
<li>Injury to the facial nerve in an accident – such as a cut to the cheek or skull base fracture.</li>
</ul>
 
Complications 
 
Around three in 10 people with Bell’s palsy will continue to experience weakness in their facial muscles, and two in 10 will be left with a more serious long-term problem. 
 
Complications include:

<li>Difficulty with speech, eating and drinking</li>
<li>Reduced sense of taste</li>

Previous medical history, was taken to determine the patient's primary problem, such as difficulty reading, blurred vision, burning sensation in the eyes, wet eyes, double vision, spots behind the eyes, or the loss of a solitary vision area (scotoma, myopia, hyperopia). Nurses must determine whether the problem is confined to one or two eyes and how long the patient has been suffering from this disorder. 
 
Also it is important to explore the state, or common ocular status of the patient.

<li>Is the patient received regular care of an ophthalmologist?</li>
